2020 Printable Contest Entry Forms
Save time at the show!

For you convenience, listed below are printable entry forms to shorten your time when registering in person at the show. 

1. Simply print out two copies of the entry form.

2.Complete the information on both forms.

3.Bring both forms along with your entry to the show.
​
4. Submit one form and your entry to ODCCA
**Make sure that your contest registration number is noted on both forms.
​
5. ​Keep the other form as a receipt to pick up your entry at the end of the show, 3 p.m. Sunday ​
